Video Summary
Dr. Scott Tomar, a dental public health professor at the University of Illinois Chicago, presented a CE webinar on community water fluoridation (CWF), covering its history, effectiveness, cost savings, and recent controversies about safety. He defined fluoridation as adjusting naturally occurring fluoride in water to an optimal level to prevent dental caries. Reviewing early 1900s observations of “Colorado brown stain” (later called fluorosis), he explained how research by McKay, Black, Dean, and Klein linked high fluoride to fluorosis but also to lower caries, leading to the “sweet spot” near 1 ppm and the first large fluoridation trials beginning in 1945. Those early studies showed dramatic caries reductions, driving widespread adoption; by 2022, about 73% of Americans on public water systems received fluoridated water.

Tomar addressed the 2024 Cochrane review, noting that strict inclusion criteria yielded few studies and led to media misinterpretations. He emphasized that fluoridation remains effective: while absolute differences are smaller today (due to lower baseline caries and other fluoride sources), relative reductions are still about 25–30% in children. He highlighted strong supportive evidence from cross-sectional studies and emphasized economics: national estimates suggest CWF saves about $6.8 billion annually (~20:1 ROI). Evidence from Calgary, Israel, and Juneau indicates caries and treatment costs rise after stopping CWF.

On safety, he stated the only credible adverse effect at U.S. levels is mild dental fluorosis. He criticized the National Toxicology Program report on fluoride and IQ as relying heavily on low-quality studies from high-exposure regions, and noted multiple high-quality cohort studies in fluoridated countries show no IQ differences. He discussed recent U.S. policy fallout, including fluoridation bans in Utah and Florida, and offered communication tips for addressing patient skepticism while respecting choice.
